Expansion of High Fidelity Wraparound Program

CCN is thrilled to share that our High Fidelity Wraparound program is expanding!

Since July 2021, CCN has been providing this program in Lake County.  In early 2023, CCN was notified that its grant application was chosen as part of the program expansion.  CCN will begin serving Kane, DuPage, Will, Champaign, Coles, Douglas, Moultrie, and Shelby counties.  Lake County will continue to be served in this program as well.

CCN’s service providers in this expansion will be SGA Youth & Family Services and One Hope United.  Through this expansion, CCN will triple its capacity in this program, moving from serving 45 clients at any given time to 135!

CCN’s Wraparound programs works with youth and families to address their unmet needs through a support system (called their “Family Team”).  To find out more about the program, please visit this High Fidelity Wraparound program page!
