High Fidelity Wraparound Program
CCN’s High Fidelity Wraparound Program facilitates the Wraparound process with children, youth, and families that are involved with the child welfare system and have complex emotional, behavioral, or mental health needs.
How it Works
Through the Wraparound process, CCN’s Wraparound Facilitators coordinate and facilitate (or co-facilitate with the caseworker) monthly Family Team Meetings to collaboratively discuss needs, strategies, and ensure alignment on the plan of care amongst informal support systems (such as family, friends, and fictive kin), and the varying formal supports (such as caseworker, therapist, and service providers) working with the enrolled child/youth.
The team accesses resources and services through community-based resources and referrals or the use of flexible funding, when necessary.
Our goals
The Wraparound process promotes permanency by maintaining and strengthening families to:
Prevent substitute care placement
Promote family reunification
Stabilize foster care placements
Facilitate child/youth development
Ensure the safety, permanency and wellbeing of children/youth
This program’s Wraparound process is expected to last 9-12 months.
Eligibility
An open Intact or Permanency case with the Illinois Department of Children and Family Services (DCFS)
Complex emotional, behavioral, or mental health needs based on the results of a Child and Adolescent Needs and Strengths (CANS) Assessment completed by the Wraparound Facilitator
Live in, or have a DCFS legal county, within the counties served (see below)
